Los Altos Hills County Fire District Video and Media Release Authorization

I grant to the Los Altos Hills County Fire District (LAHCFD or District) all rights to the use of my image, in video or still, and of the likeness and sound of my voice as recorded on audio or video, and/or the rights of any photographs or videos I have provided to the District for use in any educational or promotional materials produced by the District. I understand that my image, and/or photos or videos may be edited, copied, exhibited, published, or distributed by the District or its designees and waive the right to inspect or approve the finished product wherein my likeness appears.

Additionally, I waive any right to royalties or other compensation related to the use of my image or recording.

By signing this release, I acknowledge that I have completely read and fully understand the above release and agree to be bound thereby. I release any and all claims against the District or its designees arising from the use of this material for educational or promotional purposes.

This authorization is effective as of the date of signature and applicable to District event and activities from the date of signature through the end of the calendar year.
